Title: Proactive Pharmacists: Decreasing Fall Risk in Elderly Patients 65 Years Old or Older

   

With an ever-increasing elderly patient population (65 years old and older) it is important as members of the healthcare team to identify and decrease the prescription medications that cause fall risks in the elderly population. There are many different lists to identify these medications, such as Beers, STOPP, and NCQA. Each of these lists have their own benefits and uses. Of particular interest, the Centers for Medicare and Medicaid Services (CMS) uses the NCQA/Hedis list to evaluate healthcare providers’ performance.

1. Differentiate current medication lists used to decrease high fall risk in elderly patients
2. Identify current Centers for Medicare and Medicaid Services (CMS) pay for performance requirements
3. Describe strategies that pharmacists can use to help decrease prescription medications causing a high fall risk in elderly patients
