Join us for this encore presentation of a continuing education program presented at PPA's 2014 Annual Conference!

 

Park Nicollet Health Services began incorporating pharmacists in the Patient Centered Medical Home in 2011 and was named a CMS Pioneer ACO in 2012. The future of healthcare is team based. Safe and effective medication use is a cornerstone to treatment of chronic disease. Pharmacist services are embedded and valued in all layers of this integrated health system. Working with leaders across the organization, pharmacists are impacting patient care and have the data to prove it. Regardless of your practice setting you will be inspired about the future of the pharmacy profession.

4. Identify stakeholders and champions to propel efforts to integrate pharmacists in the ACO and PCMH models.
