Title: Cost Justifying the Pharmacist as a Member of the Healthcare Team: Documentation beyond the SOAP Note
This program will start with a discussion of the current state of pharmacists as members of the healthcare team in various settings and why we need to be able to justify our positions. Many hospital and ambulatory care pharmacists document in SOAP notes and have limited ability to search or report on their interventions. Conversely, community pharmacists are restricted to documenting prescription errors and brief interventions, and are often not documenting their clinical thoughts or interventions. This discussion will explore available options for pharmacists in any setting and allow participants to brainstorm documentation strategies for their practice settings.

2. Describe three data collection platforms available for pharmacists’ documentation.
3. Identify relevant data to collect based on practitioner and administrative input.
4. Theorize strategies for increasing feasibility of tracking interventions.
