Title: Opioid Wars: The Conversion Strikes Back

   

Inappropriate dosing of opioid pain medications has led to numerous overdoses and at times death in both the inpatient and outpatient setting.  In practice, several health care providers can be unaware of key concepts, conversion factors and appropriate dosing strategies for these high risk medications. Many people fear opioid conversions due to a lack of understanding and, at times, difficult math behind each solution.  However, it does not have to be this way. The aim of this program is to simplify the complex and convoluted world of opioid conversions regardless of practice setting in an effort to improve patient safety.

Target Audience: All pharmacists with routine interaction or questions of opioid pain medications

CE Credit: 1.5 Contact Hours (0.15 CEUs)

Program Objectives:
At the completion of this activity, the participant will be able to:
1. Recommend appropriate dosing strategies for opioid naïve and tolerant patients
2. Recognize when to compensate for incomplete cross tolerance
3. Calculate appropriate doses of IV, PO, long and short acting opioids
4. Evaluate the safety of opioid doses given a specific patient case
5. Discuss the nuances in converting to and from fentanyl patches and methadone
