Title: From Hospital to Home: Ensuring Safe, Effective, and Affordable Use of Target-Specific Oral Anticoagulants for Acute Venous Thromboembolism


Target-specific oral anticoagulants are being utilized at an increasing rate due to support from national guidelines, new options for reversal, and proven safety and efficacy compared to standard treatment. Transitioning a patient from hospital to home on these agents can be complicated and potentially dangerous. This program will provide both health-system and community pharmacists with the transitions of care tools needed to ensure safe, effective, and affordable use of the target-specific oral anticoagulants for treatment of acute venous thromboembolism.


Jacqueline Theodorou, PharmD, BCPS, Clinical Assistant Professor, Temple University School of Pharmacy

Dr. Theodorou served as the Clinical Pharmacy Specialist in Anticoagulation at Pennsylvania Hospital, where she created Pennsylvania Hospital’s first Inpatient Anticoagulation Management Program. Her duties included overseeing anticoagulation therapy, collaborating with various medical teams to manage high-risk patients, and providing education regarding anticoagulation therapy to physicians, nursing, and pharmacy staff. She also played a vital role in the Stroke Certification Task Force, which helped Pennsylvania Hospital successfully achieve accreditation as a Primary Stroke Center from the Joint Commission in 2012. In 2013, Dr. Theodorou became a Clinical Assistant Professor at Temple University School of Pharmacy. In addition to her teaching duties, she currently serves as a preceptor for the Inpatient Clinical Advanced Pharmacy Practice Experience rotation on an Internal Medicine service at Pennsylvania Hospital, which offers opportunities for transitions in care interventions for patients that are initiated on oral anticoagulants. She has also recently published a case report regarding recurrent venous thromboembolism in two cancer patients taking rivaroxaban. 


Target Audience: Health system pharmacists and community pharmacists

CE Credit: 1.5 Contact Hours (0.15 CEU)

Program Objectives:
At the completion of this activity, the participant will be able to:
1. List the appropriate venous thromboembolism treatment dosing regimens for target-specific oral anticoagulants.
2. Describe patient-specific factors to consider prior to initiation of target-specific oral anticoagulants.
3. If given a patient case, develop a safe, effective, and affordable care plan upon transition from hospital to home.
