Transitions of Care Programs

Transitions of Care Programs and References

This page is at a starting point of collecting various helpful references, program examples, best practices, and other potential helpful resources, for pharmacists interested in the ever-growing topic of managing transitions of care (TOC) for patients.  TOC programs can involve managing care in patients transition from hospital to community, community to long term care, hospital to long term care, and any combination in-between. PPA in partnership with PSHP has established a work group which has been working on providing educational and informational resources, collecting best practices, and other ways to address this issue for the past two years.  Currently the work group is working on a white paper and developing a possible research project, among other things.  For more information on the members of the work group, please click here.
