Fighting the Good Fight: How to Convert Opioids Just Right!
In this interactive case-based session, the audience will learn the intricacies of converting between different opioids, including between routes and different agents. Considerations about agent selection, dosing, as well as nuances of clinical practice will be discussed. Case-based learning will be used to illustrate conversion concepts and will offer many opportunities to practice!

Tanya J. Uritsky, PharmD, BCPS, CPE Clinical Pharmacy Specialist Pain Medication Stewardship, Hospital of the University of Pennsylvania
Dr. Uritsky is a Clinical Pharmacy Specialist in Pain Medication Stewardship at the Hospital of the University of Pennsylvania (HUP). She was a founding member of the Palliative Care Service at HUP. She has published literature on the subject of pain and symptom management and speaks nationally and internationally on the topic. Dr. Uritsky received her Doctor of Pharmacy degree from the University of Maryland, Baltimore. She completed an ASHP-accredited Pharmacy Practice Residency at Union Memorial Hospital and an ASHP-accredited Specialty Residency in Pain and Palliative Care through the University of Maryland School of Pharmacy, Baltimore. She is Board Certified in Pharmacotherapy and is also a Certified Pain Educator. She serves as Chair for the ASHP Special Advisory Group for Pain and Palliative Care. She was awarded Palliative Care Practitioner of the Year in 2015 by PainWeek and Preceptor of the Year at HUP in 2017

1. Assess whether a patient is an appropriate candidate for switching among opioids.
2. Describe how to calculate opioid dosages when switching patients among opioids.
3. List factors for consideration when switching patients among opioids.
4. Given a case, accurately perform an opioid calculation between routes and between different opioid analgesics
