Treating Patients with Penicillin Allergies: A Review of the True Incidence of Cross-Reactivity

 

One of the major roles of a pharmacist is to evaluate a patient’s allergies and to recommend avoiding medications which may cause a patient to experience an adverse reaction. Traditionally, pharmacists have warned physicians that in a patient with a penicillin allergy, the rate of cross reactivity to cephalosporins is 10%; however, the literature suggests a much lower incidence. This presentation will review recent literature surrounding penicillin allergies, cross reactivity, and the negative outcomes associated with a penicillin allergy listed on a patient’s profile.

Pamela J. McCormick, PharmD, BCPS, Clinical Pharmacist, UPMC Mercy Department of Emergency Medicine

Pam (Lyons) McCormick graduated from the University of Notre Dame with a bachelor of Science in Biology and completed her PharmD at the University of Pittsburgh. She completed her PGY1 residency at UPMC Mercy and stayed on to work as a staff pharmacist. She now works as a clinical pharmacist in the emergency department at UPMC Mercy. She is an acitve member of the Mercy Pharmacy Quality Improvement Committee and the UPMC Mercy Antimicrobial Stewardship Committee. She is a board certified pharmacotherapy specialist and maintains certification in Advanced Cardiac Life Support and Emergency Neurological Life Support

At the completion of this activity, the participant will be able to:

1. Describe types of penicillin allergies
2. Evaluate the literature surrounding penicillin allergies and cross-reactivity with cephalosporins
3. List factors which increase or decrease risk of an allergic reaction
4. Discuss how penicillin allergies negatively impact patient care
