Medication Synchronization with a (Helical) Twist: Incorporating Medication Management into a Medication Synchronization Program

 

Medication synchronization, or the alignment of a patient’s chronic prescriptions to one pick-up date each month, has been linked to a positive impact on patient adherence and persistence to a prescribed drug regimen. During this continuing education program we will discuss one medication synchronization program that has incorporated elements of medication management into the workflow, allowing the pharmacist to provide additional targeted interventions beyond simply convenient fills.

Danielle Mayer is an Assistant Professor in the Department of Pharmacy Practice at Jefferson College of Pharmacy in Philadelphia, Pennsylvania. Her current practice site is an outpatient pharmacy, where she precepts both IPPE and APPE students. Danielle earned her Doctor of Pharmacy degree from the University Of Pittsburgh School Of Pharmacy and went on to complete a community pharmacy residency with Acme Sav-On Pharmacy and Temple University, in Philadelphia.

a. Discuss the benefits of medication synchronization pertaining to the patient and pharmacy.
