DPP Best Practices Toolkit

This toolkit was created to help community pharmacies get ready to launch and continue to offer a successful Diabetes Prevention Program in their pharmacy. The tools below represent in part some of the valuable lessons learned over the last few years, in working to have a number of pharmacies in Pennsylvania obtain and remain recognized by Centers for Disease Control (CDC) for their DPP program.



This toolkit will provide guidance, tips, and resources for program requirements, workflow, patient recruitment and retention, integrating services with other healthcare providers, referrals from providers and billing basics.






It is highly encouraged that pharmacies interested in offering DPP, review the Centers for Disease Control and Prevention Diabetes Prevention Recognition Program Standards and Operating Procedures Manual (March 2018)

Standards for CDC recognition include (from CDC website)

  • Use of a CDC-approved curriculum
  • Ability to begin offering the lifestyle program within 6 months of receiving approval from CDC
  • Capacity and commitment to deliver the program over a least 1 year, including at least 16 sessions during the first 6 months and at least 6 session during the last 6 months
  • Ability to submit data on participants' progress - including attendance, weight loss, and physical activity - every 6 months
  • Trained lifestyle coaches who can help build participants' skills and confidence to make lasting lifestyle changes
  • Designated individual(s) to serve as the diabetes prevention program coordinator
  • In addition, the recognition program requires that at least 35% of participants be diagnosed with prediabetes through blood testing (or have a history of gestational diabetes)


The National Diabetes Prevention Program (National DPP) overall goal is to reduce the risk of type 2 diabetes through a partnership of public and private organizations.
