Our Vision:

Development of sustainable practice models where pharmacists and physicians collaborate in primary care throughout Pennsylvania

Our Mission:

PC2 is a group of pharmacists in primary care throughout PA that collaborates to provide networking, innovation, support, mentorship, a scholarly home for collaboration to publish our work, and a consolidated voice for advocacy. We share a standardized philosophy of practice and patient experience, and continually strive to identify meaningful outcome measures, both qualitative and quantitative, in a variety of health system models.

We Aim to:
  • Define core pharmacist role regardless of practice setting, create a united voice, identify pharmacy champions/partners.
  • Forge new relationships with stakeholders in primary care with the goal of enabling innovative initiatives in the ambulatory pharmacy space.
  • Create a directory of primary care pharmacists in PA.
  • Provide an online forum to share ideas and ask questions.
  • Outline practical solutions to expand services in different settings.
  • Get buy-in from payers through a new connection between pharmacists in the practices to payors.
  • Establish standardized philosophy and process of care.
  • Share and collaborate on templates and other resources.
  • Advocate for technician certification and work in primary care.
  • Achieve provider status in health systems, municipalities, and payers.
  • Create a strategic plan and workflow to support expansion.
  • Be an active leader in a professional organization providing input and service to the practice of pharmacy!
Join Us Today!

PC2 Membership is open to all Pennsylvania Pharmacists Association (PPA) members who are involved in or interested in primary care. You may work in ambulatory care, community pharmacy, a clinic, a healthsystem, or just about anywhere, as long as you feel a connection to primary care delivery, you are invited to join us.

We offer opportunities to:

- Collaborate, network and learn with like-minded, forward thinking pharmacists
- Develop, showcase and share innovative practice models
- Advocate, lead, and support emerging concepts in professional practice
- Innovate, promote, and deliver meaningful patient care

We accomplish this through:
  • Conference calls and discussion forums (also recorded and shared)
  • At least two annual in person meetings of programs and discussions in conjunction with PPA
  • Quarterly best practices and discussion webinar forums
  • Resource sharing
  • Providing you with an audience and opportunity in which to present or publish

How do I join?

If you are already a PPA member, simply send an email to PPA at ppa@papharmacists.com stating you wish to be part of the PC2 group.

Not a PPA member – give PPA a chance – Please consider joining our efforts by becoming a PPA member!  Once you join PPA we encourage you to email us and request being part of PC2.
