Clinical Programs for Patient Care by Pharmacists 

Pharmacists are often considered the most accessible health care providers and are thus provided with a unique opportunity to have an effect on our patient’s health outcomes. As the scope of pharmacy practice continues to head in a more clinical direction, our unique cognitive services are beginning to be recognized for how valuable they really can be. Across the nation, pharmacies are implementing programs that allow pharmacists to be reimbursed for a specific type of counseling or intervention that they are able to provide.

The Pennsylvania Pharmacists Educational Foundation is excited to announce the opportunity for up to four grants of up to $1,000 for the exploration of innovative and best pharmacy practices, especially focusing on patient centered outcomes. For more information, click here.

Patient Care Toolkits

The Pennsylvania Pharmacists Association, through our Comprehensive Medication Management Collaborative, is providing toolkits to help all pharmacists implement successful patient care programs.  The toolkits contain a variety of samples, ideas, steps, and links to help you in your development of a program at your pharmacy.  Additionally, our Mentor Collaborative members are available to try and help you with your individual needs.

CMS Informational Bulletin

This guidance addresses flexibilities that states may have to facilitate timely access to specific drugs by expanding the scope of practice and services that can be provided by pharmacists, including dispensing drugs based on their own independently initiated prescriptions, collaborative practice agreements (CPA) with other licensed prescribing healthcare providers like physicians, “standing orders” issued by the state, or other predetermined protocols. These practices can facilitate easier access to medically necessary and time-sensitive drugs for Medicaid beneficiaries.
