Toolkit: Transitions of Care Management


Transitions of Care is just one area where pharmacists can play an important role!
Patients who leave the hospital are at a high risk of complications and readmissions, much of which could be prevented through improved transitional care.  With medications being a major cause of preventable rehospitalizations, pharmacists have a large part to play in reducing post discharge complications and avoidable rehospitalizations.  Below is a list of tools to help you care for patients recently hospitatlized.
