Pharmacists' Patient Care Toolkits



The Pennsylvania Pharmacists Association, through our Comprehensive Medication Management Collaborative, is providing the following toolkits to help all pharmacists implement successful patient care programs. The toolkits contain a variety of samples, ideas, steps, and links to help you in your development of a program at your pharmacy. Additionally, our Mentor Collaborative members are available to try and help you with your individual needs.

Medication Management and Patient Care

This section is a great starting place in developing any patient care program.  It contains general tips and suggestions along with a variety of helpful resources applicable for any specific disease area or in general medication management.  The categories listed below contain resources specific to that area. 



Specific Disease State Toolkits

PPA has also developed more specific toolkits in each of the area listed below, click on any of these disease states for a listing of tools which will include training programs, guidelines, clinical resources, tools for patients, apps, and more specific to that disease state.
