Delivering Enhanced Patient Care - Part 2

Until further notice, in-person live trainings have been suspended.  instead, PPA will meet the demand for this training through live webinars.  Attendees must be able to be present for the entire time at their computer as attentiveness will be monitored and must be at 90% or higher. this means attendees cannot be doing email or other activities and are to be active participants.  Computer must have microphone capabilities or attendees must be logged in by computer and telephone.   Having a web cam is highly desirable.

 

Target Audience:

Pharmacists interested in providing enhanced patient care services having completed the online training Delivering Enhanced Patient Care - Part 1, provided through the Comprehensive Medication Management Collaborative (CMMC).

Program Objectives:

At the completion of this activity, the participant will be able to:

1. Describe the pharmacist's role in the evolving health care environment.
2. Compare and contrast the different key stakeholder engagement strategies for Comprehensive Medication
Management (CMM).
3. Analyze drug related problems associated with the CMM Case and provide clinical rationale for each drug
related problem.
4. Practice documentation needed for CMM compensation opportunities.
5. Create your own implementation plan for patient care program development
