United Healthcare and OptumRx
To participate in the UnitedHealthcare Medicaid plan, please click here for the information. The first 4 listed steps are the same as listed on the PPA website to get enrolled with medical assistance fee-for-services, a required pre-requisite. The fifth step is specific to Unitedhealthcare.
Keystone First
To participate with Keystone First, complete Steps 1-3 as outlined on PPA’s website for Tobacco Cessation Medicaid Billing and Training. Complete Keystone First’s Contract Application as a tobacco cessation provider and email the contract form to [email protected].
